Subclinical vasculitis in polymyalgia rheumatica.

چکیده
Polymyalgia rheumatica (PMR) is an inflammatory condition of the aging population characterised by pain, stiVness, and symmetrical involvement of shoulder and pelvic girdles. It has been proposed that the primary site of disease may reside outside the synovial joint, 2 but its aetiopathogenesis remains ill understood. There is good indirect evidence that vasculitis is important in the pathogenesis of PMR, based on observations from the closely related disease, giant cell arteritis (GCA). Many patients with GCA have PMR-like symptoms and the converse is also true. One of the characteristics of PMR is its dramatic response to moderate doses of corticosteroid treatment. However, a subset of patients responds inadequately to treatment or has diYculty with dose reduction, the basis for which is not well defined. We report here the case of a patient with PMR which was diYcult to treat who was found to have underlying vasculitis on magnetic resonance imaging (MRI), a finding which explains the requirement for the higher dose of steroids.

Peripheral arterial disease in polymyalgia rheumatica
Patients with polymyalgia rheumatica have been shown to have an increased risk of peripheral arterial disease on longitudinal follow-up. Possible explanations for this include premature atherosclerosis related to chronic inflammation, as with other inflammatory rheumatological conditions. Alternatively, polymyalgia rheumatica can be associated with vasculitis, even in the absence of clinical gi...

متن کاملLocalized Mesenteric Vasculitis in a Patient with Polymyalgia Rheumatica
Polymyalgia rheumatica (PMR) is an uncommon disorder characterized by bilateral pain and stiffness in the shoulder and pelvic girdles. Polymyalgia rheumatica and giant cell arteritis (GCA) occur in the same patient population and share a common pathogenesis. Giant cell arteritis predominantly affects the cranial arteries and rarely involves the gastrointestinal tract. Moreover, giant cell arter...
متن کاملGiant cell arteritis and polymyalgia rheumatica: usefulness of vascular magnetic resonance imaging studies in the diagnosis of aortitis.
OBJECTIVES One of the unresolved challenges posed in giant cell (temporal) arteritis (GCA) is the detection and monitoring of large-artery complications, particularly aortitis. Recent investigations support vascular magnetic resonance imaging (MRI) studies in this issue. We report our preliminary experience with this imaging technique in the study of the aorta and its proximal branches in patie...
متن کاملPrevalence of rheumatic manifestations and antineutrophil cytoplasmic antibodies in haematological malignancies. A prospective study.
OBJECTIVE To evaluate the prevalence of antineutrophil cytoplasmic antibodies (ANCA) and rheumatic manifestations associated with chronic haematological malignancies. METHODS Two groups of patients were prospectively studied (group I: 60 patients with myelodysplastic syndromes and group II: 140 patients with lymphoid malignancies) for clinical 'immune' manifestations and ANCA. RESULTS In th...
